The comedian later expanded on that decision while speaking on the red carpet at the 2026 Tony Awards in New York City.
O’Donnell explained that remaining transparent felt especially important at a time when she believes honesty is increasingly rare.
“Authenticity is the goal in these days and times, and people are lying about everything all day to the American public,” she told E! News. “It’s very depressing to me and unsettling, and I think all that matters is truth and love, and so I wanted to be truthful and say all the complicated emotions I had about it.”
The actress had already discussed the procedure on her Substack, where she admitted feeling conflicted about her decision.
Rather than allow speculation to take over, she preferred to speak openly about the experience and the emotions that came with it.

While many celebrities keep cosmetic procedures private, O’Donnell’s decision to go public was complicated by the reactions of people closest to her.
One particularly memorable response came from her 13-year-old child, whose opinion caused her to delay the procedure.
“My little autistic 13-year-old said to me, ‘If you do it, I would not be able to respect you ever again,’” she recalled. “And that put me off it for a few months, but then when I went and did it in January, I came home 10 days later, and they never noticed.”
The comment stayed with her long after it was made. Still, after weighing her feelings for months, O’Donnell moved forward with the surgery.
She also admitted that part of her motivation for speaking publicly was practical. Rather than wait for a tabloid to reveal the story, she preferred to tell it herself.
“I just wanted to say, ‘This is what I did, and here’s the doctor, and if you want to,’” she shared. “But it’s very expensive — it’s more expensive than any car I ever bought, but I can’t drive around in my face.”
Weight Loss Played A Major Role
The procedure was not about chasing perfection, according to O’Donnell. Instead, it addressed changes she experienced after significant weight loss.
The former talk show host revealed she had lost more than 50 pounds while taking Mounjaro and managing diabetes. That transformation left her dealing with loose skin around her face.
“I’m on Mounjaro over the last three years, I have diabetes too, and I lost over 50 pounds, and that was responsible for a lot of the extra skin that I had around my face,” she told E! News.
O’Donnell has also made it clear that she does not intend to continue pursuing cosmetic procedures.
She viewed the facelift as a solution to concerns she had about her appearance rather than the beginning of a larger transformation.
The surgery focused primarily on tightening loose skin and addressing facial lines that had become more noticeable after her weight loss journey.

After recovering, O’Donnell decided to show the results without filters or heavy editing. She posted before-and-after images and reflected on how she felt when she looked in the mirror.
“I wanted to still be me, just … less haunted,” she wrote on Substack. “And I do look like me — A slightly more well-rested, emotionally stable version of me.”
The comedian stressed that the procedure did not change who she was. Instead, she viewed it as a way of feeling more comfortable with the face staring back at her each day.
“I didn’t disappear, I didn’t become someone else — I just stopped arguing with the mirror,” she added. “And maybe that’s enough. Or at the very least … it’s what a lower deep plane face lift looks like when it minds its own business.”
Those comments resonated with many readers who appreciated her candid description of aging, self-image, and personal choice.
Public Reaction Was Deeply Divided
As often happens with celebrity transformations, reactions online were mixed.
As The Blast reported, some critics questioned the results and openly mocked the procedure after Rosie O’Donnell posted comparison photos on social media. Others argued that cosmetic surgery rarely produces the desired outcome and suggested the facelift had not made a significant difference.
